We provide compassionate care, expert symptom management, volunteer services as well as spiritual or social work support.
For our nurses, Hospice-at- Home offers flexible scheduling and autonomy, the chance to educate as you provide care, and one-on-one relationships with patients and their families.
Hospice, Registered Nurse is responsible for performing direct patient care, including evaluation of needs and conduction of case and resource management, under the direction of a licensed physician. Provides consistently high quality care to patients in accordance with the plan of care and Agency policies. Uses appropriate case management skills: makes the initial evaluation visit; regularly reevaluates the patient's nursing needs; initiates the plan of care and necessary revisions; furnishes those services requiring substantial and specialized nursing skill, initiates appropriate preventive and rehabilitative nursing procedures; prepares clinical and progress notes; coordinates services; customizes care to patient's needs, informing physician, nurse manager, other personnel, and family verbally and in writing as needed regarding changes and progress; counsels the patient and family in meeting nursing and related needs; maintains and enhances nursing skills on a regular basis and participates in in-service programs; and supervises and teaches other nursing personnel. Handles medical emergencies calmly and quickly. Communicates regularly with other disciplines involved in care, and evaluates needs for other services, making recommendations when appropriate. Delegates duties appropriately to LPNs and CNAs. Submits accurate and timely documentation of care to appropriate Agency personnel, using laptop computer or correct paperwork. Documents inter-disciplinary communication regarding patients by entering it into medical record. Charts progress accurately, thoroughly and legibly. Meets regulatory agency and pay source requirements to include goals, interventions, outcomes, frequency/duration of care, and other required information. Graduation from an approved educational program for Nursing. Must have an active Colorado RN license or a license from the state of primary residence, if participating in the NLC (Nurse Licensure Compact). Previous Hospice or Home care experience preferred. Current CPR required. Twelve months hospital-based nursing experience as an RN strongly preferred. Six months nursing experience in the last three years, or an acceptable refresher course. Home health experience preferred. Other requirements may vary by case or department needs. Anyone with less than 12 months hospital-based nursing may be subject to an extended probationary review period. Adequate nursing knowledge to provide high quality nursing care in an autonomous home care environment, without regular on-site supervision. Understands the concept of working with teams for the enhancement of patient care and of Agency operations. Psychomotor skills to provide nursing care, including repetitive pinching, grasping, and manipulating. Standing, squatting, walking, bending, and maneuvering in private homes, which are not barrier-free. Heavy lifting (greater than fifty pounds) to transfer or turn an adult, with or without assistive devices.
