BridgePoint Supportive ! Care

Purpose:
Responsible for providing a clinically advanced level of assessment and interventions to a caseload of patients enrolled in the BridgePoint Supportive Care Palliative Care Program.

Reports to:
BridgePoint Supportive Care Program Director

Qualifications:
Currently licensed as an Advanced Practice Nurse in the State of Colorado.

Currently holds nationally recognized advance practice certification

by the American Association of Nurse Practitioners or the American Nurse Credentialing Center.

Holds prescriptive authority as granted by the Colorado Board of Nursing.

Holds active DEA authority to prescribe medications.

Has a minimum of 10 (ten) years’ experience as a Registered Nurse or Nurse Practitioner.

Has excellent written and oral communication skills.

Has excellent interpersonal skills

Demonstrated ability to manage the administ! rative (time management, self-starter, etc.) and clinical aspe! cts of palliative care.

Demonstrated ability to learn quickly, remain current on end-of-life and palliative care trends, and practices utilizing evidence-based approaches.

Responsibilities (including, but not limited to):
·

Clinical â€" 75%

o

Provides clinical consultation to palliative patients at the request of the attending physician. Completes a thorough assessment, makes recommendations for palliative interventions and communicates to attending physician; follows-up as necessary.

o

Develops plan of care for palliative patients and communicates with patient’s attending physician. Prescribes interventions as appropriate.

o

Communicates with, educates and supports patient’s loved ones as needed.

o

Documents care provided adhering to industry standards and submits to Medical Records for processing in a timely manner.

Education â€" 5%
o

Provides clinical education to the PeopleFirst Hospice and Home Care staff

o

Provides community education as requested.

o

Acts as a clinical resource for agency staff as requested.

o

Maintains compliance with agency and industry standards for continuing education

·

Administrative â€" 5%

o

Participates as requested in quality monitoring and interventions agency-wide.

o

Submits bills for services provided in a timely manner according to program policy and payor policy.

o

Complies with all pertinent state and federal regulations.

o

Creates and maintains relationships with vendors, referral sources and the area-wide medical community at large.
