NURSE CASE MANAGER

Description: Exempla Healthcare is currently seeking to employ NURSE CASE MANAGER right now, this position will be placed in Colorado. Facilitate the effective management of patients in collaboration with Physicians and other members of the health care team on the patient's behalf, ensuring that the patient is receiving care in the app! ropriate setting at the right time, and in the most cost effective manner. Enhances the quality of patient care through effective and efficient use of resources. Improves organization and coordination of care to specific patient groups.
MINIMUM QUALIFICATIONS: " Graduate of an accredited school of nursing " Minimum of three years clinical nursing experience " Knowledge and experience with managed care In addition to the responsibilities described above, the role may include such other responsibilities and duties as assigned from time to time, based upon Exempla needs or requirements.
PREFERRED QUALIFICATIONS: " One year of case management experience " BSN
