 To provide direct support to clients in an integrated behavioral health/primary care program located in a communit!   y mental health center 
 Essential Duties:
 Manages data entry and tracking to support disease management and stepped care approach 
Dispenses medications and maintains Medication Administration Records 
Actively provides case management support for wellness services and basic needs 
Conducts psycho-educational groups and wellness classes in conjunction with other staff 
Provides education to staff regarding an integrated BHPC model 
Provides supportive counseling to clients regarding their targeted treatment action plans 
Tracks laboratory data 
Obtains vital signs 
Works with prescribers to help monitor health and follow up of clients 
Obtains healthcare information from other healthcare providers 
Outreaches to Federally Qualified Health Centers (FQHC) and other healthcare providers 
Schedules appointments with other FQHCs 
Completes documentation of services or case notes consistent with clinical and administrative po!   licies and procedures 
Promotes and demonstrates MHPs missi!   on, vision and values through both behavior and job performance on a day to day basis 
Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc 
Regular and consistent attendance is required to perform other essential functions of the job 
 Minimal Qualifications:
 Bachelors Degree in Psychology, Social Work, Counseling, or related field OR HS Diploma/GED with active Certified Nursing Assistant certificate 
QMAP training and certification required. If not certified, training will be required immediately upon hire 
Prior experience in direct client care in a behavioral health, hospital, homeless shelter or other community agency setting preferred 
Must have reliable transportation and willingness to travel from site to site 
 Knowledge, Skills and Abilities:
 Understanding of the integrated model and basic knowledge of physical & behavioral health diagnoses 
Ability to work well with a dive!   rse group of individuals and to apply principles of cultural responsiveness 
Self-directed and able to work independently using sound judgment 
Computer proficiency in MS Office (MS Word and Outlook) and familiarity with electronic health records 
Strong team player 
Excellent interpersonal, verbal and written communication skills 
Knowledge of community resources and ability to make appropriate referrals 
Excellent problem solving skills 
Ability to work independently as well as in a team setting
