Salary Range is $24.45 - $34.45 per hour DOQ and Budget approval, with excellent benefits.
Seeking a Generalist Public Heal th Nurse that is primarily responsible for the coordination and implementation of the Immunization Clinic, HCP Children with Special Health Care Needs Program and assisting with Disease Control investigation activities. This position includes direct patient care in clinic and assists with back up coverage in disease investigations as needed. This nurse will provide care coordination to assist families and children with special health care needs in identifying and navigating community resources. Additionally, this position will occasionally work on general Public Health and Maternal Child Health activities as assigned. The ideal candidate will thrive in a fun work environment that has a variety of responsibilities and will exhibit flexibility while working on multiple programs in a small office setting. The incumbent works in partnership with other employees, departments/divisions, external entities, and the public in delivering effective and innovative services. Provide holi stic services to internal and external customers by seeking ways to integrate programs or services provided by other departments, divisions, and external entities.

General Duties:
Administers immunizations, provides education on vaccines and safety as appropriate. Assists with Clinic inventory, maintenance and immunization outreach events. Performs health assessments of clients, identifies health care needs an d resources, and develops effective treatment plans for clients. Provides care coordination for clients and children with special health care needs and assist with identifying community resources, early intervention education and referrals. Adheres to and helps develop program policies and protocols. Provides education and resources to the community and external partners. Assists in preparation of promotional materials and may conduct presentations to improve community awareness of public health services. Assists with public health calls and prioritizing tasks. May assist with creating and implementing additional public health programs.
Collaborates and develops professional contacts with other Health and Human Services divisions, county agencies, community agencies, non-profit organizations, and vendors to effectively serve the needs of the citizens. Provides programmatic technical assistance and training to community agencies and providers, such as schools, child care providers or senior centers as needed. Attends meetings, workshops, and conventions as necessary for the maintenance of effective services. Follows policies, procedures, and standards for efficient and effective operation and maintenance of equipment. Operates a motor vehicle and personal computer with applicable software. Keeps advised of the current status of the work of the department, as well as pertinent new rules, regulations, legislation, procedures, and professional developments. May assist with reproductive health clinic as necessary; performs applicable laboratory work. Performs related duties as required.
Administrative Duties:
The incumbent is responsible for effectively managing his or her workload to achieve departmental goals. Assures that responsibilities are performed in compliance with federal and state regulations, Broomfield policies and procedures and the Broomfield Municipal Code. Maintains confidentiality of information consistent w ith applicable federal, state and county rules and regulations. The incumbent may confer with employees, supervisory, and administrative personnel regarding departmental policies and procedures. Contributes to the preparation and review of departmental policies and procedures as necessary to maintain the effectiveness of departmental programs. Advises applicable administrators or departments of potential problems or concerns. Compiles, interprets, and prepares data for studies, reports and recommendations; coordinate departmental activities with other departments and agencies as needed.
The incumbent assists in recommending and implementing modifications to systems, policies and procedures. May conduct studies, analyses, and research on a wide variety of topics. Assists in researching and analyzing legal requirements impacting departmental programs and services. Makes recommendations on changes or improvements.
Budget/Resource Management:
The incumben t assists the Public Health Nursing Administrator in the preparation of the department's annual budget. Assures that responsibilities are performed within budget and performs cost control activities such as monitoring expenditures to assure sound fiscal control. The incumbent is responsible for partnering with other staff members in planning, developing, and implementing the overall vision, mission, programs, processes and projects of the department. Works with the team to avoid duplication of service and to ensure that customer expectations are recognized and resolved thereby maximizing customer satisfaction. Assists in the development of short and long-range goals including annual efforts to advance the City and County's mission, goals, and objectives. Monitors the progress of projects so the goals may be obtained within the established timeframe. Prepares purchase orders and processes invoices as assigned.
Public Relations/Communication:
The incumbent meets and confers with customers to explain departmental plans, programs, functions, policies, and procedures. Assists departmental staff in preparing and coordinating reports as required. Answers letters of inquiry/complaints and talk with customers. Attends meetings, workshops, and conventions as necessary for the maintenance of effective services. Responds to requests for information and advises City and County departments on assigned programs. Maintains liaison with other departments. Provides verbal and written information to departments. Maintains the accuracy of any program materials posted on the Intranet/Internet.
Miscellaneous Duties:
Respond, as required, to support public health emergencies, incidents and events. Participate in all exercises and drills on emergency preparedness, as required. Complete trainings identified as appropriate for this level employee. Assist with implementation of the public health improvement plan (PHIP) as appropria te. Maintain confidentiality of information consistent with applicable federal, state and city/county rules and regulations. Operate a personal computer including associated software programs. Operate a motor vehicle in traveling to and from work sites. Take proper safety precautions, anticipate unsafe circumstances, and act accordingly to prevent accidents. Use all required safety equipment. Perform related duties as required. The incumbent keeps operational records and makes required reports. Assures that facilities and equipment are maintained properly and coordinate maintenance and repair functions. Evaluates service costs and makes related recommendations. Identifies and recommends improvements in departmental operations to ensure assigned functions are efficient and cost effective. Conducting home visits, if applicable may involve carrying a case filled with materials such as a small DVD player, files, notebook, educational materials, toys, a baby scale, etc., and walk ing up and down stairs frequently when no elevator is available.
Minimum Qualifications:
Training - Bachelor's degree in nursing from an accredited college or university.
Experience - Three years of increasingly responsible nursing experience, preferably in public health, with some responsibility in implementation of community health programs. Some experience working with special needs population is preferable. Bi-lingual skills including Spanish are helpful but not required.

NECESSARY SPECIAL REQUIREMENTS: - Licensure to practice professional nursing in the State of Colorado. CPR and first aid certifications; Basic Life Support is required. Must be at least 21 years of age. Must possess and maintain a valid driver's license and safe driving record for continued employment. New hires must, as a condition of employment, pass the following pre-offer and post-offer/hire processes: oral board, reference checks, background checks including local police check, child abuse and sex-offender registry, CVSA or polygraph, driving check through DMV, and fingerprint checks through CBI/FBI.
Working Conditions:
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job. The noise level in the environment is moderate. The work schedule is varied and the incumbent must be available to work any day of the week including evening, weekend and holiday hours. The incumbent must be on-call and available to respond to emergencies within a reasonable time frame with little or no notice. The incumbent may meet with clients in their homes or other environments, requiring travel by motor vehicle. Conducting home visits may involve carrying a case filled with materials such as a small DVD player, files, note book, educational materials, toys, a baby scale, etc., and walking up and down stairs frequently when no elevator is available.
