Description: The Denver Hospice is at the momment seeking for Nurse Practitioner-Palliative Care Consults right now, this position will be placed in Colorado. Detailed specification about this position opportunity please read the description below. POSITION SUMMARY:
Conducts consults based on patient’s individual needs including: pain and symptom management, FFE's, goal settin! g and clarification of treatment options, advanced care planning and discussion regarding illness and end of life issues. Confers with collaborating physician, as needed, for assistance with making recommendations for patient’s plan of care. Provides appropriate recommendations and documentation. Works in collaboration with other members of the palliative care team. Provides timely follow-up and feedback to patients, providers and other members of healthcare team.
KNOWLEDGE :
- Knowledge of hospice and palliative care philosophy
- Knowledge of medical documentation process
- Knowledge of documentation and billing procedures for provider level contacts
Able to provide advanced nursing care as required by state/federal regulations and certification
Able to provide advanced nursing assessments and documentation
Able to refer to other members of the palliative care team
Able to communicate ! compassionately and effectively with team members, patient/fam! ily and community
Able to educate patient/family and community about hospice and palliative care philosophy and appropriate care
Able to display flexibility, independence, initiative, creativity, confidentiality, integrity, stability, and efficiency
EDUCATION/CERTIFICATE/LICENSE:
Current Colorado RN license, and advanced practice registry
Valid Colorado state drivers license (auto, $100,000 Liability), with qualifying driving record
Board certified as Adult or Family Nurse Practitioner by the AANP or ANCC
Current BCLS certification
Prescriptive authority and DEA number (preferred)
EXPERIENCE:
MINIMUM :
1 year experience as RN in med/surg, public health, oncology, acute care, or home health
1 year of palliative care/hospice training, either as student or professional
