Nurse Practitioner

Description: Life Choice Hospice is presently looking of Nurse Practitioner right now, this position will be placed in Colorado. The Hospice Nurse Practitioner (HNP) provides health care to patients and their families through the application of advanced knowledge and skills. The HNP provides consultation in palliative care, sym! ptom management and supportive care to meet the needs of the patient In collaboration with the Hospice Medical Director, the Nurse Practitioner provides services related to health palliative/end of life care. The HNP works collaboratively within an interdisciplinary health care team and acts as resource person for staff and others on care related issues.
Job Qualifications:
Experience
Minimum of one year of experience as a Nurse Practitioner required. Minimum of five years experience as a Registered Nurse required; background in hospice, home care, critical care, oncology or medical/surgical nursing preferred. Experience with Suncoast is highly preferred.
Education
Graduate of an accredited school of nursing required from a state-approved school of nursing accredited by either the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). MSN required. Licensed as a Regi! stered Nurse in the state of practice. Current certification b! y a national certifying body as a nurse practitioner with prescriptive ability and a current DEA certificate. Certification as a Hospice and Palliative Nurse in highly preferred.
Special Qualifications
Must possess excellent assessment, diagnosis and treatment skills. Must be highly motivated, energetic and work interdependently. Requires excellent written and oral communication skills, basic time management and organizational skills, strong interpersonal relationship skills, and must be supportive of organizational philosophy/policies. Must possess a valid and current state driver’s license and automobile insurance. Must have reliable transportation. Must be a creative problem solver.
