Manages psychiatric and chemical dependency facility-based and outpatient professional treatment through telephonic case management. Primary duties may include, but are not limited to: Using appropriate screening criteria, knowledge and clinical judgment assesses patient needs and assures that medically necessary treatment is provided in a quality, cost-effective manner. Follows guidelines to refer cases to Medical Advisors as appropriate. Utilizes skills and knowledge in psychiatric and chemical dependency assessment, negotiation, coordination, implementation, case planning, monitoring, and evaluating to promote quality member outcomes, to optimize member benefits, and to promote effective use of resources. Ensures member access to medical necessary, quality beha! vioral healthcare in a cost effective setting according to con! tract. Additional duties include but are not limited to:
Performs telephonic care management within the scope of licensure for members with behavioral health care needs.
Conducts assessments to identify individual needs.
Identifies specific care management plan to address objectives and goals as identified during assessment.
Coordinates internal and external resources available to meet identified needs.
Uses health coaching and motivational interviewing to identify members willingness to change. Monitors and evaluates effectiveness of care plan.
Modifies care management plans as necessary.
Assists in problem solving providers, coordination with existing providers and support into appropriate treatment.
Job Qualifications
Requires a Masters degree in social work, counseling or related behavioral health field, and a minimum of 3 years of clinical experience in Social Work, or Counseling with curr! ent, unrestricted license as a Clinical Social Worker (LCSW), Professional Counselor (LPC), Marriage and Family Therapist (MFT). Experience in case management and telephonic coaching with patients with a broad range of complex psychiatric and or medical disorders is required. Excellent communication and interpersonal skills are essential. Self motivation and the ability to assume a lead role in ensuring that all objectives are met is desired. Strong computer skills are required. Bi-lingual in Spanish preferred.
