Responsibilities Uses computerized scheduling system to assign new referrals to caregivers on basis of patient needs, location, caregiver skills, and caseload. Maintains and updates scheduling system accurately and efficiently. Ensures continuity of care, reassigning caregivers as needed and notifying supervisor of conflicts. Apply online
