Participates in admission and discharge planning for residents, performing an initial written evaluation of resident after admission and completes follow up evaluations thereafter.
Actively involved in the resident care plan team and promotes harmonious working relations within the community.
Actively promotes and raises awareness of the facility and company principles to civic, community, and Alzheimer’s related groups within the community! . The candidate of choice will have the following skills and b! ackground:
Master’s Degree in Social Work or Gerontology is preferred, Bachelor's Degree may be considered.
Several years experience working with Medicare, Medicaid and current knowledge of insurance updates.
Excellent reading, writing and oral communication skills.
