SUMMARY: The night and evening registered nurse is specially trained in the hospice concept to provide on-call services for the personal, physical and emotional needs of the patient and family. Duties may include triaging calls, making routine and urgent visits, presentations and admissions, and attending deaths. The after hours nurse may be responsible for patient care in any setting, home, nursing home, assisted living, hospital or Hospice Care Center.
ASSESSMENT AND NURSING INTERVENTION: Accurately assesses patient or family needs and intervenes appropriately .
1. Consistently performs skilled nursing assessment
2. Develops appropriate plan of action to resolve problems arising after hours
3. Evaluates resources available to patient, and coordinates with the on call social worker, chaplain, and other team members when patient/family require additional support
4. Assesses learning needs/styles and inclu! des patient/family teaching and goals as part of the Nursing i! ntervention.
5. Performs treatments/interventions in accordance with the patientâs plan of care and according to physicianâs orders
6. Effectively addresses the patient/family needs while changing the plan of care as little as possible though changes the POC to meet the needs of patients/families.
7. Assesses for appropriate level of care for patient/family needs/problems and Initiates a change in level of care when appropriate (GIP, Continuous Care, Respite) Completes appropriate paperwork.
PROVISION OF CARE: Provides quality hospice nursing care while demonstrating effective organizational and time management skills.
1. Consistently responds to on call pages within 15 minutes.
2. effectively triages calls, determining the appropriate intervention and type of intervention (call or visit) needed to address the need/problem in a timely manner.
3. Approaches all calls with the belief that the patient / family has a need / problem with wh! ich they need assistance and works to meet those needs to the best of their ability during their assigned shift.
4. Makes tuck-in calls and visits to all newly admitted patients and to any patient who has experienced any change or problem when requested by a team member.
5. Makes visits to all patients admitted to a hospital or an emergency room (Except VA) during their Shift to ensure patients are receiving care according to their wishes.
6. Arranges for medications and / or supplies needed during the evening or night.
7. Assists the HCC staff with the care of the patients in the inpatient unit when not busy with Community calls and visits.
8. Allows time during visits and calls to provide supportive counseling within the realm of nursing practice.
9. Makes an effort to provide all requested visits without the use of back up whenever possible through utilizing on-call staff (back up nurse, social worker, chaplain, C.N.A, hospice physician and Admini! strator on call) to ensure patients and families get the services they ! need.
COORDINATES CARE THROUGH COMMUNICATION & COLLABORATION: Effectively coordinates the appropriate care of patients/families during the evening/night hours by collaborating and communicating with the IDT
1. Turns in accurate activity log for each shift within the required time frame.
2. Completes documentation in a thorough and understandable manner for each call and/or visit which paints the picture of why the patient/family needed services and what services/interventions they received and the outcome of those interventions.
3. Calls Physician (attending and / or hospice physician) for orders and changes in plan of care.
4. Physicians orders are written accurately and legibly and profiled with Hospice Pharmacia.
5. Effectively communicates with internal and external customers to get the necessary services and resources for the patient/family.
6. Leaves accurate and detailed message via voice mail or email for clinical manager and patie! ntâs team of problems/needs/changes which occurred during the night.
SELF DIRECTED & RESPONSIBLE BEHAVIOR: Stays informed and educated in hospice regulations and hospice nursing practice. Effectively works within the team and within the community, presenting self in a friendly and professional manner at all times.
1. Has a thorough understanding of and practices according to all HPCWCâs policies and procedures.
2. Actively involved in problem solving and quality improvement regarding any identified after
hours issues / concerns, including revision of policy and procedures.

7. Has obtained Hospice & Palliative ! Care certification within 2 years of employment with HPCWC.

9. Understands and adheres to the process of ordering medications and DME.
10. Demonstrates knowledge of fiscal responsibilities in planning and delivering care, utilizing supplies, DME and medications in a cost effective manner.
11. Displays courtesy, tact and patience for all internal and external customers.
12. Promotes a positive image throughout the community, through appropriate dress and grooming, language, and behaving in a customer service friendly manner.
13. Demonstrates ability to solve problems independently, while seeking consultation when needed.
14. Accepts responsibility and accountability for own actions.
15. Accepts feedback in a non-defensive manner.
16. Addresses any conflict with supervisor and / or co-workers in an honest, open and appropriate manner, seeking assistance to resolve issues when needed.
QUALIFICATIONS:
- RN with! current Colorado licensure; graduate from an accredited school of nursing.
- One year Hospice or three years of medical/surgical, oncology, or comparable hospital experience.
- Valid Colorado driverâs license and unlimited access to reliable transportation.
- Adequate automobile insurance.
- Ability to set priorities and follow through.
- Participates in the on-call, holiday, and weekend and after hours programs.
- Reads, writes and speaks English in a manner that can be understood by others.
- Medium Work â" exerting 20 to 50 pounds of force occasionally (i.e. up to 1/3 of the time) to move objects, including the human body. Frequent walking and standing.
- Standing/Walking â" must stand/walk frequently and intermittently to accomplish tasks. May also choose to stand while completing clerical duties.
- Lifting â" must frequently raise objects, including the hum! an body, from a lower to a higher position or move objects, including t! he human body, horizontally from position to position.
- Pushing/Pulling â" must frequently use upper extremities to push and/or pull objects, including the human body, in a sustained motion.

- Fingering/Grasping â" must occasionally finger and/or grasp objects, including the human body, during the provi! sion of patient care.
- Feeling â" must occasionally touch objects, including the human body, to perceive attributes such as size, shape, temperature, or textures.
- Talking â" must frequently exchange information in person and on the telephone to patients, doctors, family members, and co-workers by means of spoken word.
