The Social Worker provides Medical Social Services to patients/families by a qualified social worker under the direction of a physician and in accordance with the plan of care.
! ESSENTIAL DUTIES AND RESPONSIBILITIES:
Include the following; however other duties may be assigned:
Assess emotional factors related to terminal illness.
Assist the physician and other IDT/IDG members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.
Assess the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect.
Assess environmental resources and obstacles to maintaining safety.
Participate in development and revision of the plan of care.
Provide social services including: Short-term individual counseling; Crisis intervention; Assist in providing information and preparation of advance directives; Funeral planning issues and transfer or responsibilities regarding fiscal, legal and health care decisions.
Prepare clinical notes.
Identify family dynamics and communica! tion patterns.
Involve the patient/family in the plan! of care..
Identify and utilize appropriate community resources and assess patient/family ability to access them.
Participate in in-service programs.
Evaluate patient/family response to psychosocial interventions.
Assess need for counseling related to risk assessment for pathological grief.
Assess special needs related to cultural diversity including communication, space, role of family members and special traditions.
Identify the developmental level of patient/family and obstacles of learning or ability to participate in care of patient.
Address patient/family questions and issues.
Identify obstacles to compliance and assist in understanding goals of interventions.
Identify support systems available to reduce stress and facilitate coping with end of life care.
Evaluate for long-term care when appropriate and assess ability to accept change in level of care. Communicate psychosocial in! formation to inpatient facility when level of care is changed.
Assist patient/family in assessing financial resources when appropriate.
Identify patient/family needs when discharged or when level of care changes.
Evaluate response to interventions when referred to a community agency and satisfaction with the service provided.
Assess Bereavement needs.
Participate in the QAPI hospice performance improvement program.

Required Experience
EDUCATION AND EXPERIENCE, SKILLS, CERTIFICATES, LICENSES, AND/OR REGISTRATIONS:
Education: Bachelors Degree in Social Work. Meets state requirements for position.
Experience and Skills: Two years of experience in hospice, home health care, or long-! term care, with grief counseling specialty preferred. Basic computer skills
Certificates/Licenses/and/or Registrations, etc..: Licensing requirements vary by state in which they work.
Other Requirements: Valid driver's license and evidence of current auto liability insurance TB test, meet new hire and State requirements for position. Understands the practices and procedures relating to referral of victims of abuse/neglect/mistreatment/exploitation to the appropriate community agencies. Promotes Community education and awareness of hospice services. Understands and maintains professional boundaries in relationship with the patient/family/caregiver. Displays sensitivity to issues of loss and grief. Has the ability to deal effectively with the demands of the job.
WORK ENVIRONMENT:
Work environment varies but includes private homes, nursing homes, assisted living centers, and continuous care facilities. Exposed to outside weat! her environment due to travel. It is a stressful environment meeting th! e emotional and spiritual needs of patients and families facing terminal illness and death.
