Under the direction and supervision of the Clinic Services Manager, the Licensed Practical Nurse (LPN) provides skilled nursing as it pertains to the Clinic for participants of InnovAge Greater Colorado PACE (IGCP).
! Essential Functions and Work Duties
The following essential functions of this position are performed personally, in cooperation with your supervisor, and/or in coordination with other staff. Additional work functions and duties may be assigned.
LICENSED PRACTICAL NURSE - CLINIC
- Gathers data for the triaging of participants, to determine priority of care. Maintains timely flow of participants by assisting other staff in clinic regarding daily appointments and emergencies.
- Provides general nursing care which includes the supervision or administration of prescribed medications and treatments in accordance with nursing standards.
- Prepares equipment and aids physician during treatment, examination, and testing of participants.
- Performs participant treatment(s) per policies and procedures.
- Ensures that wound care orders are up-to-date in EMR and updates the wound care tracker as needed. Communicates with facilities in rega! rd to wound care measurements.
- Implements the orders ! written by primary care and nursing orders when necessary.
- Assists with coordinating specialist appointments.
- Performs phlebotomy functions for clinic when necessary and identifies and labels specimens appropriately.
- Instructs participants and family regarding medications and treatment instructions and ensures understanding of given information.
- Ensures that medications for participants are packaged, labeled and ready for delivery by the Pharmacy Courier in a timely manner, when appropriate.
- Tracks medication receipts received from the Pharmacy Courier and communicates with the pharmacy to ensure that participants received their medication(s) in a timely manner.
- Observes, records, and reports participantâs condition and reaction to drugs and treatments to physicians.
- Schedules specialist appointments and assists in hospital admissions and calling report to unit, assigned or requested.
- Signs off on ord! ers, and explains orders and instructions to appropriate personnel, implements orders written by primary care and nursing when necessary.
- Maintains exam rooms for necessary supplies and materials and ensures cleanliness. Lists clinic supplies that need re-stocking on the supply list as appropriate.
- Gathers data for the nursing assessment form on visitors and on participants every four months, as assigned or requested.
- Administers participant and employee vaccinations and TB testing, documents and records results test administration and results appropriately.

Education and Experience
Licensed Pr! actical Nurse â" Clinic
- Current Colorado Licensed Practic! al Nurse (LPN) license required.
- One to three years of experience as an LPN in a clinic or long term care setting with geriatrics experience and IV Certification is preferred.
- Current CPR card or the ability to obtain a CPR card is required.
- A minimum of one year of experience working with the frail or elderly is required.
