Clinical Manager--Optio Health Services

Description: The Denver Hospice is presently looking of Clinical Manager--Optio Health Services right now, this position will be placed in Colorado. Responsible for the supervision of the clinical team(s) to assure the implementation of patient care is in accordance with established contracts and the stan! dard of care outlined within the company, and when applicable, in accordance with Federal and State Regulation. Assembles and maintains the clinical team and demonstrates leadership skills within the organization as well as in the day to day team activities. Communicates clearly and expresses self well in groups and one on one conversation. Responds as a leader to departmental needs and participates in multi-departmental groups to establish/revise processes and procedures that accommodate the functioning of the department/team. Promotes and models excellence in the delivery of care and customer service in all areas of the work environment to include but not be limited to; work colleagues, vendors, community agencies, partners, patient and family. Participates in clinical on-call duties and provides back up to Clinical Managers, as needed. Demonstrates problem resolution attributes and able to confront issues that are impacting the department negatively.

LICENSES/EDUCATION :
Active Colorado LSW/LCSW or RN license
RN must have or be actively seeking a Bachelorâs Degree in nursing or related field such as health care administration or business
Required Skills
Knowledge and ability to apply/consult with referral sources, partners, and collegues with disease trajectory and clinical assessment/appropriate intervention in a variety of arenas of care â" palliative care, advanced illness management that including but not limited to physical, psychosocial, spiritual needs.
Understand and support role of the nurse practitioner in the palliative care/hospice environment and able to address concerns/issues with the physician (medical director)
Successful experience in areas of leadership, finance, budget, complex care management, risk assessment, community resources, quality outcomes, relationships in the workplace, customer service, and contract partnerships.!
Demonstrates ability to problem solve independently with collegues and customers
Has experience in managing office staff, clinical professionals and providers.
Able to balance work load and prioritize multiple tasks on a daily basis.
Able to shift focus easily through work day and able to handle sensitive issues with thoughtful regard and prioritization of action/outcome.
Detail oriented and quality experience a plus
Clinical experience in the home environment a plus
Required Experience
4+ years experience in health care field
3+ years managing a multidisciplinary team
Familiar with Home Health Visit Model and ability to transcribe knowledge to the HVP and HCC assessment process.
Previous experience working and collaborating in a team environment
Home Health experience is necessary for Home Health Regulated palliative care services.
