PASCO is growing rapidly and is looking for an experienced and compassionate Field Skilled Nurse (R.N.) to join our team! Our Field Nurse will act as a comprehensive care coordinator for consumers
assigned to his / her care, continually assessing the consumerâs needs,
coordinating care amongst all providers and ensuring high quality care. Perform skilled nursing services, provide consumer education, complete and submit all required documentation, and
supervise and direct assigned care providers.
Key Duties and Responsibilities include:
- Establish and maintain relationships with assigned consumers
- Perform comprehensive assessments of physical, emotional, psychosocial, and environmental needs of the consumer including outcome and assessment information
- Develop and implement a realistic individualized Plan of Care (Physicianâs Plan of Care, Home Health Aide Plan of Care) for each consumer, evaluate and revise this plan as necessary; interpret the appropriate information needed to identify each consumerâs requirements
- Participate in development of consumer goals as well as make revisions to these goals based on the changes in consumer status.
- Interpret and implement physicians orders accurately and promptly and demonstrate appropriate procedures as necessary to LPNs, CNAs or other care providers
- Monitor visit frequency and duration by reviewing physiciansâ orders and comparing to the Plan of Care and documentation
- Identify significant clinical findings, make conclusions and intervene appropriately
- Review results of laboratory tests and other diagnostic procedures and report to other care provider appropriately
- On an ongoing basis, evaluate the effectiveness and outcomes of care and the appropriateness of the Plan of Care, and initiate necessary revisions with communication with all other disciplines
- When unable to handle care (e.g. vacation or leave), make attempts to find a replacement and communicate status to Supervisor
- Complete and submit all required documentation per governing body rules and regulations and within established Agency guidelines
- Proactively schedule and perform consumer visits to meet deadlines
- Prioritize nursing visits based on the consumerâs health status and anticipated needs.
- Perform skilled nursing services under the orders of the consumerâs physician and in accordance with accepted standards of practice and governing body rules and regulations
- Supervise and direct assigned LPNâs and aides per Federal, State and Local regulations and Agency Policy.

- 1 year experience practicing as an RN required, experience in acute care, medical / surgical nursing strongly preferred

- Current license to practice as a Registered Nurse in the state of Colorado
- A Bachelors in Nursing, Associates in Nursing or Diploma in Nursing required
- Current CPR / First Aid certification
- Clinical Judgment: Ability to independently collect and interpret data about a consumer, arrive at a conclusion and identify the appropriate action. This involves problem solving, decision making and critical thinking rooted in clinical knowledge and experience.
